I ask parents to fill out this form along with the lesson agreement. It helps me get to know your child and be a more effective teacher.

  • display appropriate behavior
  • has the discipline for self-guided practice

Fine Motor Skills

The skills here will help you know whether your child has the dexterity needed to play the piano effectively.

  • knows left and right
  • lift fingers independently
  • print letters
  • use scissors
  • button shirt
  • grasp pencil

Mental Skills

  • can count to 10 and do basic addition
  • knows ABCs, has early reading skills
  • can keep a steady, clapped beat
  • recognizes symbols and can transfer meaning

  • owns an instrument
  • time commitment
  • cost
